    REPORT AN ABSENCE

    PK-8 Parents MUST CALL 973-584-0812 x104 or EMAIL the School Nurse by 8:00AM to report a child’s absence and its reason. Use the above button to launch your email application and report your student absence. NOTIFICATION TO THE NURSE OFFICE IS REQUIRED.

    • Texting/emailing/leaving a voicemail with a Teacher or the Main Office does NOT fulfill the above requirement. You WILL receive a call from the Nurse's Office if your student is absent and you have NOT SPECIFICALLY notified the Nurse’s Office (this includes vacations, etc.). This is a security protocol.
    • If a student is absent for three or more consecutive days, a Doctor’s note MUST ACCOMPANY the child back to school.
    • If a student is recorded as “absent”, they may not attend any campus activities e.g. clubs/events on that recorded “absent” day.

  • ANNUAL PHYSICAL REQUIREMENT

    St. Therese School requires that ALL students have an ANNUAL physical and the parent/guardian MUST submit current, required documents to the Nurse’s Office. The NJ State Universal Child Health Record with Immunizations Record attached, completed and signed by the student’s physician, MUST be on file in the Nurse’s Office annually.
    • DOCUMENTS ARE ONLY VALID FOR ONE YEAR FROM THE DATE OF THE PHYSICAL.
    • Students who fail to have a completed health form and immunization record will not be permitted into the school until the form is submitted.

    MEDICATION ADMINISTRATION DURING SCHOOL HOURS

    New Jersey State law PROHIBITS administration of ANY MEDICATION, Prescription and/or any “over-the-counter” (OTC) medication  e.g. Tylenol, Advil, Benadryl, without a DOCTOR’S ORDER AND THE PARENT/GUARDIAN’S WRITTEN CONSENT.

    • All Medication MUST BE DELIVERED to the school nurse by a parent or guardian. NO MEDICATION can be brought in by a student and any medication brought in by a student will not be administered.
    • All OTC medications MUST be supplied from home and labeled with the student’s name.
    • Students MAY NOT CARRY ANY MEDICATION ON THEIR PERSON for the purpose of self-administration. There are exceptions to that rule: i.e. Inhalers and Epi-Pens for life-threatening conditions such as asthma, bee stings, and/or food allergies. Inhalers and Epi-pens may only be carried by students who demonstrate appropriate knowledge of technique and administration. A doctor’s order and parent/guardian written consent is REQUIRED.
    • ANY Medication administered in school MUST be in its original container and labeled with the student’s name.
    • A fax (973-584-2029) will be accepted from the prescribing physician, and parent or guardian, if your child needs to take medication in school. It is the parent’s responsibility to call the doctor to request a medication order.
    • If medication is a controlled substance the number of pills must be verified with the school nurse.
    • Prescriptions must be written for the CURRENT school year only, and will not carry over to the following school year.
    • All unused medication should be collected by the parent/guardian at the end of the school year and if not, it will be properly discarded.
    • MEDICATION SENT TO SCHOOL IN A BAGGIE, ENVELOPE OR TISSUE WILL NOT BE ADMINISTERED.
    • STUDENTS ARE NEVER PERMITTED TO SELF-ADMINISTER MEDICATION.

    ASTHMA TREATMENT PLAN

    Students with asthma may carry their inhalers with them only with physician authorization. Your physician may write on a prescription or on office letterhead that 1. your child is responsible, 2. has been instructed in the proper use of the inhaler and 3. may carry his/her inhaler with him/her at all times. This note/letter must be on file in the nurse’s office along with the Medication Administration Form and Asthma Treatment Plan.

    ALLERGY CARE PLAN

    Students with severe food and insect sting allergies, who have EPI-PENS prescribed by their physicians may require the assistance in the absence of the school nurse, to administer their epi-pen. We have specially trained and certified staff members who may be assigned to assist such students. However, parent/guardian must contact the school nurse to discuss their child’s condition and request a designee for the epi-pen in the absence of the school nurse. Documentation must be completed by the parent and the child’s physician and must be on file in the nurse’s office along with the FARE Form and Medication Administration Form.
